When you get a new follower you receive a notification and the user profile pic is saved to an album

When a person follows you on twitter, there will be a notification sent out to you with all the details and also the persons profile picture will be saved to a special IFTTT created photo album (iOS only!)

This Trigger fires every time a new user starts following you.

Then

Notifications icon
Send a rich notification from the IFTTT app

This action will send a rich notification to your devices from the IFTTT app. Rich notifications may include a title, image, and link that opens in a browser or installed app.

And

iOS Photos icon
Add photo to album

This Action will save a new photo to the album you specify.
